1. Print out and complete the simple application form (see below)
2. Bring the form into any of our branches
3. Citibank will contact you once they received your application - it's that easy!

Non Members

You may only apply for a Citibank MyCard MasterCard if you are a current member of SERVICE ONE Members Banking.

Becoming our member is easy!
